
MFUGE is almost here!
In order to go to camp, we need you to fill out the following forms and submit them. Some of these forms are submitted straight to MFUGE and others are submitted to Bethlehem Baptist Church. You must have these forms done in order to attend camp, so please get them filled out ASAP!!!

Camp Waiver & Release Form
Every person attending camp must complete and submit a Camp Waiver & Release. Parents of minors should complete the form. Group leaders and adult sponsors will complete their own forms.
Ridgecrest Recreation Waiver
Every person attending camp must complete and submit the Ridgecrest Recreation Waiver. This allows participants to participate in activities in and around the Ridgecrest MFUGE Camp facilities. Please fill this out and send a scanned or photographed copy
to students.director@gmail.com or give the signed copy to Brett in person.
Bethlehem Medical Release Form
Every person attending camp must complete and submit the Bethlehem Baptist Church 2023 medical release form. If you have completed this form previously, we ask that you still complete this form so we can have updated information.
Adult Leader forms

Child Protection
Training
All adults attending camp must complete our Child Protection training module. This consists of 5 videos to watch and a test to take at the end. Once this is complete, print your completion certificate and give it to Brett either by giving your printed copy or sending a scanned or picture copy of the completed form to students.director@gmail.com.
