MFUGE is almost here!
In order to go to camp, we need you to fill out the following forms and submit them. Some of these forms are submitted straight to MFUGE and others are submitted to Bethlehem Baptist Church. You must have these forms done in order to attend camp, so please get them filled out ASAP!!!

Students Forms
When asked for the Registration ID, use 83997
Camp Waiver & Release Form
Every person attending camp must complete and submit a Camp Waiver & Release. Parents of minors should complete the form. Group leaders and adult sponsors will complete their own forms.
Liberty Recreation Waiver
Every person attending camp must complete the Liberty University Recreation Waiver. This allows participants to participate in activities in and around the Liberty MFUGE Camp facilities. Please fill this out and send a scanned or photographed copy
to brett.hay@bbcmidlo.org or give the signed copy to Brett in person.
Rock Wall Risk Assumption Form
Every person who would like to have an opportunity to climb the rock wall at Liberty's LaHaye Recreation and Fitness Center must complete this form and send the completed copy to brett.hay@bbcmidlo.org or give the signed copy to Brett in person.
Snowflex Equipment Rental Agreement
Snowflex is Liberty's synthetic tubing, skiing, and snowboarding recreation center. It allows participants to enjoy winter actives all year. In order for students to be allowed to participate in these activities, each student must complete the Snowflex Equipment Rental Agreement Form and send it to brett.hay@bbcmidlo.org or give a completed copy to Brett in person.
Adult Leader forms

Child Protection
Training
All adults attending camp must complete our Child Protection training module. This consists of 5 videos to watch and a test to take at the end. Once this is complete, print your completion certificate and give it to Brett either by giving your printed copy or sending a scanned or picture copy of the completed form to students.director@gmail.com.
